At Family Councils Ontario (FCO), the past year has been one of meaningful progress, collaboration, and renewed purpose. Our 2024–2025 Annual Report captures a pivotal moment in our journey—one where we strengthened our role as a provincial partner, expanded our reach, and deepened our commitment to families and caregivers in long-term care.

Throughout the year, we focused on building stronger Family Councils across Ontario through education, coaching, and connection. From launching new initiatives like the Administrator Forum to providing one-on-one support to councils navigating complex challenges, our work has remained grounded in relationships and real-world impact.

A standout achievement this year was the continued success of our Navigation Guide to Long-Term Care. Designed with caregivers, residents, and sector partners, this resource is already helping families approach the transition into long-term care with greater clarity and confidence.

We also took an important step forward through our Innoweave Strategic Clarity process—refining how we define and measure impact. This work is shaping a clear roadmap for the future, including the development of our Champion model, expanded inclusive supports, and stronger data-driven decision-making.

This year was also a time of celebration. As we marked 10 years as an independent organization, we reflected on our growth from a grassroots initiative to a province-wide organization supporting hundreds of Family Councils and thousands of caregivers.

Looking ahead, we remain focused on strengthening Family Councils, fostering collaboration, and helping create long-term care environments where residents, families, and staff can thrive together.
