Working Together to Improve Elder Care Navigation

We’re excited to share that our sector partner, Elderado, has begun working with the Durham Ontario Health Team to launch a new Elder Care Navigation section on the DOHT website. This new resource brings together practical information to support older adults, caregivers, and families, and we’re proud that it includes a link to FCO’s Making the Move to Long-Term Care guide as part of the available supports. Collaborations like this reflect the power of working together to strengthen access to clear, community-informed resources across Ontario.
